• Review and obtain last two years of claims payout history
• Prepare future Medical Cost Projection of Medicare expenses as related to the work injury
• Obtain Rated age and Life expectancy
• Assist in resolving any Medicare lien issues
• Written report which includes injury description, all CPT and ICD-9 codes, summary of medical care, current treatment plans, future treatment plans, Medicare and Social Security disability status, pre-existing/co-morbid conditions, and allocation amount
